Life Stage Transition Support
SSS Support provides life stage transition support for people with disability navigating major life changes — available through NDIS funding (Registration Group 0106) and on a private basis for individuals and families who prefer to self-fund.
Major life transitions — leaving school, moving into independent living, entering the workforce, adjusting to changed support needs — are challenging for anyone. For people with disability, they carry additional complexity. With the right support in place, what could be an overwhelming shift becomes a planned, confident step forward.
What Is Life Stage Transition Support?

Life stage transition support is structured, goal-focused assistance designed to help people with disability navigate specific major life changes. Unlike general life skills development — which builds everyday practical skills over time — transition support is focused on a particular change point and on building the connections, plans and capabilities needed to move through it successfully.
Under the NDIS, this support is funded through Registration Group 0106 — Assist-Life Stage, Transition — under Capacity Building in your plan. It is time-limited and goal-specific by design: an investment in a particular moment of change, building the foundation for a more independent future on the other side.
For individuals not funded through the NDIS, SSS Support provides the same structured transition support on a private-pay basis — particularly valuable for families planning ahead of a first NDIS application or supporting a young person through a transition independently.
What Transitions Does SSS Support Help With?
School to post-school life
Leaving school is one of the most significant transitions a young person with disability faces. The structured routines and built-in social connections of school disappear almost overnight. SSS Support works with young people and their families well before school leaving age — helping identify goals, build skills, and connect with the right services and opportunities. For NDIS participants, this can be funded through the School Leaver Employment Supports (SLES) program under 0106.
Moving out of the family home
Transitioning to independent or supported living is a major milestone — for the person and the family. We help prepare for this transition practically and emotionally — developing independent living skills, exploring accommodation options and making sure the move happens at a pace that works for everyone.
Entering employment or further education
Starting work or study for the first time introduces new demands — workplace expectations, new routines, transport, social dynamics. We support people in building the skills and confidence to navigate these environments successfully, working alongside employment and education services rather than replacing them.
Changed support needs
A new diagnosis, a significant change in functional capacity, a carer becoming unavailable — when circumstances shift significantly, transition support helps bridge the gap, plan new arrangements and land safely in a new support structure.
Ageing and transitions from NDIS to aged care
For older participants navigating the transition from NDIS to aged care services, we provide support in understanding options, navigating the system and making informed decisions. Note: the NDIS cutoff for new applicants is age 65 — existing participants can choose to remain on the NDIS.

School Leaver Employment Supports (SLES) — Start Planning Early
The NDIS School Leaver Employment Supports (SLES) program — funded under Registration Group 0106 — is specifically designed to help young people leaving school build work-related skills and explore employment options. It can fund transition support for up to two years after leaving school.
SSS Support can provide SLES-funded transition support, working alongside the participant, their family, their school and any employment service providers involved.
We recommend starting transition planning at least 12–18 months before the participant’s final year of school. The earlier a family begins this conversation, the more prepared and confident their young person will be when the time comes — and the more options will be available. Contact SSS Support to discuss early transition planning for your young person.
Listen first — we take time to understand the person’s goals, anxieties, strengths and current situation before making any recommendations.
Plan collaboratively — transition plans are developed with the individual, their family, and for NDIS participants, their support coordinator and allied health team.
Build skills in real settings — we practise transition skills in the actual environments the person will be navigating — workplaces, public transport, community spaces, new accommodation. Not theory. Real life.
Connect to the right services — a good transition means being connected to the right support on the other side. We actively help build those connections.
Stay flexible — transitions don’t always go to plan. We adapt as things change and maintain support through the full transition period.
What's the Difference Between Transition Support (0106) and Life Skills Development (0117)?
These two supports are often confused — both involve building skills, but they serve different purposes:
0117 — Development of Daily Living and Life Skills
Ongoing coaching in practical everyday skills — cooking, budgeting, communication, transport — building independence over time. Not tied to a specific life event.
0106 — Assist-Life Stage, Transition
Specifically focused on navigating a particular life change. Time-limited, goal-specific and tied to a transition point. The skills built are in service of getting through that transition successfully.
Both can work together — many participants have 0106 funding to navigate a transition and 0117 funding to build the ongoing skills they’ll need on the other side. SSS Support is registered under both groups and can deliver both supports.

Privately funded individuals and families
If you’re not yet on the NDIS, are in the process of applying, or prefer to self-fund this support, SSS Support provides the same structured transition assistance on a fee-for-service basis. Frequently Asked Questions
What is NDIS Registration Group 0106 — Assist-Life Stage Transition?
Registration Group 0106 funds support that helps people with disability navigate major life transitions — leaving school, moving out of home, entering employment, changes in support needs and ageing transitions. It sits under Capacity Building in your NDIS plan and is focused on a specific transition point rather than ongoing daily support.
Can I access life stage transition support without NDIS funding?
Yes. SSS Support provides life stage transition support on a private-pay basis for individuals and families who are not yet on the NDIS, are waiting for a plan, or prefer to self-fund. This is particularly common for families planning a school-to-adulthood transition ahead of NDIS funding being confirmed. Contact us to discuss private arrangements.
What is the NDIS School Leaver Employment Supports (SLES) program?
SLES is NDIS funding under Registration Group 0106 specifically for young people leaving school — designed to build work-related skills and explore employment options for up to two years after school. SSS Support can provide SLES-funded transition support. We recommend starting the SLES planning conversation at least 12–18 months before the participant’s final year of school.
What is the difference between life stage transition support (0106) and life skills development (0117)?
Life skills development (0117) is ongoing coaching in everyday practical skills — cooking, budgeting, communication — building independence over time. Life stage transition support (0106) is specifically focused on navigating a particular life change — time-limited, goal-specific and tied to a transition point. Both can work together and many participants have both in their NDIS plan.
When should we start planning for my child’s school-to-adulthood transition?
At least 12–18 months before the participant’s final year of school — ideally earlier. Early planning means more time to build skills, explore options and make sure the right supports are in place before school ends. Contact SSS Support to discuss early transition planning for your young person.
Is life stage transition support the same as support coordination?
No. Support coordination helps participants understand and implement their NDIS plan. Life stage transition support (0106) is hands-on, skills-focused assistance delivered directly to the participant — building the practical capabilities and connections needed to navigate a specific life change. The two work well together but serve different purposes.
